INSURANCE
I am an out of network provider and can provide statements that can be submitted to your insurance if you have a PPO plan. If you have an HMO plan, then you can ask for a "single case exception".
When calling your insurance to determine your coverage, give them the tax id/EIN number: 81-1088163, (The Institute for the Advancement of Psychotherapy, my group practice) and possibly NPI# 1447545033. Ask what your deductible is, what percentage of the fee they pay, their customary rate, and how many sessions you are allowed per year. They may ask for a procedure code, which would be 90834 for Individual Therapy or 90847 for Family or Couples Therapy. Mental health coverage varies by plan. Also, it is important to be aware that it may take the insurance company a few months to process your receipts for repayment. If you have an HMO, you can ask your insurance for a "single case exception", explaining why you are unable to find another clinician with my same skills and abilities in your area, and they may be able to cover the treatment.
Please note, I am also the director of a nonprofit, Bay Area Community Counseling, which provides services by associate therapists, so that we may serve those in financial need. BACC is in network with Aetna insurance, and I am listed on the BACC website, but it is the associates in the practice that see clients through Aetna, not myself directly.
