Insurance
I provide receipts that can be submitted to your insurance if you have a PPO plan and if you have an HMO plan, then you can ask for a "single case exception". When calling your insurance to determine your coverage, give them my tax id/EIN number: 26-4017801, ask what your deductible is, what percentage of the fee they pay, their customary rate, and how many sessions you are allowed per year. Mental health coverage varies by plan. Also, it is important to be aware that it may take the insurance company a few months to process your receipts for repayment. If you have an HMO, you can ask your insurance for a "single case exception", explaining why you are unable to find another clinician with my same skills and abilities in your area, and they may be able to cover the treatment.

I accept checks, cash, or credit cards as payments. You can pay by credit card through paypal by clicking on the "Pay Now" link. Due to paypal fees, please add $0.30 and divide by .971
